Chemo Orders and Consents
The list of disease and specialty groups below are links to available chemotherapy orders and consents frequently used in their treatment. As we strive to keep these documents up-to-date, revisions are frequent and ongoing, we ask that you always obtain a new form from this listing each time you prescribe these drugs.
If you notice any issues or have questions, please call 717-531-8678 for immediate assistance Monday through Friday, 8 a.m. to 4 p.m., or email us at chemotherapyordersets@pennstatehealth.psu.edu and we will respond to your inquiry as soon as possible.
As always, the final responsibility for drug dosing and appropriate use of these order sets, reside with the prescribing physician.
